We are a small team within Monmouthshire County Council known as the Disabled Adaptations Team based within the Social Care Department. We are dedicated to helping people live independently and safely in their home. If you are finding it difficult to get in and around your home because of poor mobility or disability, then we may be able to help you with an adaptation.

You could get a grant from your council if you’re disabled and need to make changes to your home. For example if you:

  • are physically disabled
  • have a learning disability
  • have age-related needs
  • are autistic
  • have a cognitive impairment, like dementia
  • have a progressive condition, like motor neurone disease
  • have a terminal illness
  • have a mental health condition

When your home no longer meets your mobility, or your disability and access needs have become difficult to manage, an adaptation to your home may help you. There are many types of adaptions which include stairlifts, level access showers and ramps.  These adaptations enable you to carry out day to day tasks, whilst continuing to live safely and independently in your home. Adaptations can make a big improvement to your quality of life and help meet your long-term needs at home.

The maximum amount of grant you can apply for in Wales in £36,000. Depending on the type of grant awarded and your income and/or savings, you may also have to contribute financially towards the cost of the adaptation.

To get a grant, you must:

  • be disabled; and
  • intend to live in the property during the grant period

The person who submits the application must be either the:

  • owner
  • tenant
  • landlord

The council needs to be happy that the work is:

  • necessary and appropriate to meet the disabled persons’ needs
  • reasonable and can be done, depending on the age and condition of the property

The work should be completed within 12 months of the grant being approved.

In order to start the process you will first need to have a professional assessment from an occupational therapist. They will carry out a thorough assessment and recommend the best adaptation to meet your needs. Once this has been carried out, the OT will then send a referral to us. Once we have received the referral we will be in touch with you.

However, you may need a minor adaptation to meet your needs such as a handrail or a step. These sorts of smaller adaptations may be provided under the Safety at Home Scheme which is administered by our joint working partners at Care & Repair.
