This exciting opportunity has arisen for a highly motivated, experienced and
innovative professional to join the South Monmouthshire Integrated Service Team
based at Chepstow Community Hospital.
The post offer an excellent opportunity to work with manage, supervise get alongside
a dynamic multi-disciplinary team and with a range of providers and partner
organisations to develop community based solutions in providing support to
vulnerable individuals. The successful candidate will need to be flexible, highly
motivated, innovative and enthusiastic with the ability to deliver a high quality
individualised service.
