Your answers will not change the care and support you receive from us. The unique reference number on the questionnaire is only to let us know that you have replied so we don’t ask you to complete the questionnaire again. If you do not know this number, you do not need to complete this section. We will provide anonymised answers to the Welsh Government so they can monitor how well we are providing our services. Your answers will not be shared further unless we have any serious safeguarding concerns.
