Through Care Team

The Through Care Team are responsible for the care management and support of those children and young people who are looked after by the County Council on a long term basis, the majority of whom will be cared for by foster parents within the Monmouthshire county boundary.
       
Additionally the team provide aftercare support for young people/young adults who have been " Looked after " , supporting them as they move to independence.
