HELP YOUR GP TO HELP YOU...

You can now register with your GP as a carer at the following surgeries in Monmouthshire:
  • Hereford Road, Abergavenny
  • Old Station Surgery, Abergavenny
  • Tudor Gate Surgery, Abergavenny
  • Chippenham Surgery, Monmouth
  • Dixton Road Surgery, Monmouth
  • Wye Dean Practice, Tintern
  • Raglan Surgery
  • Vauxhall Surgery, Chepstow
  • Mount Pleasant Surgery, Chepstow
Look for the form "Help Your GP To Help You" in the reception area of your surgery.  There is a second form - Carer 2 - that must be completed if you want further details added to either your record or the record of the person you are looking after.

ADVANTAGES OF REGISTERING WITH YOUR GP AS A CARER
When you are registered as a carer with your GP it means that whenever the GP accesses your record he or she can see that you have caring responsibilities. This would make them more aware of your situation and why you may be suffering from a certain condition - for example back pain can be brought on by lifting, moving and handling a patient at home.
Registration also means that locum doctors are made aware of your situation when they access your record.
It can also make it easier for your GP to contact you with information about help that is available to you as a carer - for instance to tell you if you are eligible for a 'flu jab.

The Monmouthshire GP Carers Registration Scheme gives you the opportunity:
  • To have details of your caring role put on your own record
  • To have the details of the person you are caring for included in your record
  • To have your details included on the record of the person you are  caring for
  • To request that details of a patient's medication or condition or both can be shared with you as their carer

IT IS IMPORTANT THAT IF YOU WANT TO INCLUDE THE DETAILS OF THE PERSON YOU ARE LOOKING AFTER ON YOUR RECORD OR TO PUT YOUR DETAILS ON THEIR RECORD, OR TO SHARE INFORMATION ABOUT THEM, YOU MUST OBTAIN THEIR CONSENT ON THE FORM CARER 2 WHICH IS AVAILABLE ON REQUEST FROM THE GP RECEPTIONIST, AND RETURN IT TO THE SURGERY
